Who
is USA+?
USA+ is a Provider Access Organization whereby, dentists, doctors,
hospitals, pharmacies, alternative medicine practitioners, lenscrafters,
and others offer their services at discounted rates below their
usual and customary fees.
What
are the average savings that I could expect from this Member Benefits
Package?
You will pay up to 10% to 60% on general dentist services, 20%
on dental specialists services, up to 30% on doctors and hospitals,
35% on Vision Services, up to 30% on alternative medicine, lowest
price on prescriptions, and more.
How
long do I have to wait to use these services?
The services can be used after you get your membership packet
and membership identification card in the mail, which usually
takes about 14 days. ( Ameritas Dental and VSP is not available
to the member until the beginning fo the next month, if they enrolled
April 24 they would not have these benefits until May 1)
If
I already have Dental and Medical Benefit can I use USA+ Benefits
with my insurance?
You may use the dental portion with existing dental insurance
(that all depends on the dental office), however you cannot use
the medical portion with existing medical insurance.
If
I have a pre-existing condition, can I still use this?
Yes. All pre existing conditions are accepted.
How
do I use my USA+ membership?
It's simple! Make your appointment with any of the providers or
facilities that participate in USA+. Please use the Benefits/Programs/Package.
Inform the person with whom you set your appointment that you
are a member of USA+. When the services are provided, submit your
USA+ membership card and pay the discounted negotiated rate. There
are not claims to submit because this is NOT
insurance.
(What we suggest to our members is to print out the Dear Dr.
letter. This letter is located at www.usahc.com
under provider search and make sure to take this letter to the
appointment. The letter will explain to the Dr's office how the
program works. It will also provide the Dr's office with a provider
relations phone number for additional questions.)
